I agree to immediately notify LMS/OSS should a client, representative of client or referral of client, or any individual sharing space in client office make a direct offer of temporary, part-time or full-time employment to me while on a temporary assignment or interview. I understand my obligation to notify LMS/OSS extends for a period of twelve months from the last temporary assignment date or interview referral date. It is further agreed that I will advise client of their obligation to contact LMS/OSS to review client’s financial responsibility. In the event I do not contact LMS/OSS regarding the above, I acknowledge my responsibility to pay LMS/OSS the placement fee. Additionally, should I accept a part-time or full-time position with an LMS/OSS client to which I was referred by LMS/OSS and subsequently withdraw that acceptance as a result of counteroffer or other reason; I acknowledge my responsibility to pay LMS/OSS the placement fee.

All Legal Medical Staffing/Origin Staffing Solutions employees and associates are and can be subject to pre-placement and random substance screenings. Multi-panel screenings are done to detect the presence of the following illegal substances including, but not limited to: cocaine, marijuana, opiates, amphetamines, benzodiazepines as well as cleansing and clearing agents. Any and all offers of employment maybe contingent upon a negative substance screening and can be withdrawn by LMS/OSS at anytime without prejudice.
